NJ Bonner AmeriCorps Enrollment - Part 2 - Enrollment Workbook

Step 1: Member attends NJ Bonner AmeriCorps Member Orientation

All NJ Bonner AmeriCorps members must attend an AmeriCorps Orientation.  During the orientation, you must read through the Member Contract with your Members.  During the orientation, Members should complete the Enrollment Workbook. This orientation will familiarize your Members with the rules and regulations related to AmeriCorps.  You may use the NJ Bonner AmeriCorps 101 presentation but be sure to read through the entire contract with your Members.

 

Step 2: Member Fills Out NJ Bonner AmeriCorps Enrollment Workbook During Orientation

During the NJ Bonner AmeriCorps Orientation, you will guide NJ Bonner AmeriCorps members through the process of completing their Enrollment Workbook.  Please ensure that all steps on Page 3 of the Enrollment Workbook are completed correctly.

 

Ensure Members understand that the Enrollment Workbook forms a binding legal contract, and that therefore they must strictly follow each of the following:

  • Write legibly in black or blue permanent ink.
  • Never use "white-out" or other correction fluid or correction tape.
  • If it is necessary to make a correction, run a line through the error and simply write the correction next to it. Both the Member and Site Supervisor or Campus Administrator must initial the correction.
  • Member should sign where appropriate using his/her full legal name (no abbreviations or nicknames) as shown on their eligibility documentation.
  • Ensure that wherever a dated signature is needed, the date given corresponds to the actual date signed. (Pre- or post-dated signatures will invalidate the contract and result in the Enrollment Workbook being returned for re-completion, with a detrimental effect on the Member's requested start date.
  • Ensure that the Enrollment Workbook and all signatures are dated on or before the Member's start date.

 

The Site Supervisor or Campus Director must complete the following in the Enrollment Workbook:

 

  • Member Contract: Initial at the bottom of Pages 7 - 28.
  • Member Contract: Print name, sign and date Page 28.
  • Position Description: Print name, sign and date position description.
  • Prohibited Activities Addendum: Sign and date on Site Supervisor line on Page 31.   

 


Step 3: Member Completes the Non-Harassment Policy Member Contract Addendum 

The Member must read and sign and date the Non-Harassment Policy Addendum.  The Site Supervisor or Campus Director must read, sign and date the addendum.

 

Step 4: Member Completes the NJ State Commission Criminal History Check Verification Form

  • If the Member IS cleared to serve unaccompanied prior to enrollment, the Member only needs to complete Page 1 and the headers on Pages 1 - 4.
  • If the Member IS NOT cleared to serve unaccompanied prior to enrollment, the Member must complete Page 1, the headers on Pages 1 - 4 and Page 2 must be completed as follows:

           > Site Supervisor must print name, sign and date.

          > Accompanying Staff Member must print name, sign and date.

          > Member must print name, sign and date.    

          > NJ Bonner AmeriCorps Staff at the Bonner Foundation will sign as Program Staff once the completed form reaches the Foundation office.

 

Step 5: Member Completes the W-4 Form

 

Step 6: Mail or Hand Deliver the NJ Bonner AmeriCorps Enrollment Workbook, NJ State Commission Criminal History Check Verification Form, and the W-4 Form to the Bonner Foundation.  We must receive these materials NO LATER THAN 5 DAYS AFTER THE MEMBER'S START DATE.
