FBI Criminal History Check

 

If you are a United States citizen, you must obtain your FBI criminal history check via our contracted vendor, Fieldprint.  Please ask your supervisor for the instructions.

 

If you are a United States legal permanent resident, you must obtain your FBI criminal history check directly from the FBI.  Please visit your local police station and obtain three sets of fingerprint cards.  Please save the receipt so that you can be reimbursed.

 

Turn in the fingerprint cards along with the completed FBI Applicant Information Form to your supervisor.

 

Click here to download, complete and print the Applicant Information Form.  To download the form you will be asked to indicate the reason for request.  Please indicate To Review Your Own Record.  

 

Instructions for Completing the Form:

 

1.  Complete Applicant Information and Applicant Home Address Sections

 

2.  In the Mail Results To Address Section, please put:
